Chronic Care Management is a program for Medicare patients who have 2 or more chronic conditions such as asthma, allergies, diabetes, high blood pressure, high cholesterol, high triglyceride levels, depression, cancer, COPD, dementia and autoimmune disorders. This program was created to help prevent urgent care visits, complications and rapid progression of your conditions. With chronic conditions, prevention plays a key role in management and the rate of progress of the disease.
Patients are supervised by health care professionals who provide a comprehensive care plan for the patients’ needs. Chronic care coordinators will manage all the patients appointments with their different healthcare providers, keep track of your medications, and monthly calls (or more often, depending on the patients need).
Patients have access to 24 hour assistance by their chronic care coordinator in the case of a medical emergency, health related questions, or whatever the patient requires. They also can communicate with the physicians in case of urgent matters, manage transitions between healthcare providers including referrals, preventative care, follow ups after emergency care or hospital visits.
The Chronic Care Management program (CCM) is a no-cost service that offers additional help in order to aid in the management of chronic conditions like asthma and allergies.

People with Medicare that live with two or more chronic conditions are eligible to enroll in the program at no cost.. With STAAMP´s VIP CCM program you will have a designated Care Coordinator with a deep understanding of your medical history that will be in constant communication with STAAMP´S providers to give you the utmost personalized care.
Your Care Coordinator will create a tailored plan designed to your health conditions and will provide personalized assistance to make sure you meet your health goals. With our month by month calls our team of Care Coordinators will make sure you stay on track with your medications, follow the right instructions for your health and answer your health questions so you receive the care you need.
You will be able to contact your Care Coordinator with our direct phone line, helping you to stay on track with your treatments and overall care plan.
You may pay a monthly fee, and the Part B deductible and coinsurance apply. If you have supplemental insurance, or have both Medicare and Medicaid, it may help cover the monthly fee.
